A proposal form is the basic instrument signifying the evidence of contract between the insured and the insurer.

The insurer seeks to elicit all the material information, which is required for underwriting the risk through the use of this instrument.

Important as it is for the Insurer the proposal form is equally important from the proposer / insured's point of view. All the facts stated therein shall be binding on both the parties and failure to appreciate its contents might lead to adverse consequences in the event of claim settlement.
